ABSTRACT

Objective To investigate the clinicopathological characteristics, diagnosis and treatment of huge adrenal tumor.Methods Retrospectively analysed the clinical data of the 2 patients with huge adrenal tumor more than 10 cm in diameter, who were admitted in our hospital from September 2013 to January 2015.Clinical manifestations, treatment and prognosis were evaluated.Results Two patients were both suspected of adrenocortical carcinomas.Case 1 was a 28-year-old female.Laparoscopic surgery was performed at first but was conversed to open palliative adrenalectomy due to tumor rupture and bleeding.The postoperative pathology was adrenocortical carcinoma.The patient died of gastrointestinal bleeding caused by tumor metastasis 10 months later.Case 2 was a 39-year-old female, who underwent laparoscopic adrenolectomy with tumor excision completely.The postoperative pathology showed neoplasm of malignant potential, and no tumor recurred in 15 months follow-up.ConclusionsOpen surgery should be the priority if the diameter of adrenal tumor is more than 10 cm and diagnosis as suspicious of malignancy.Huge adrenocortical carcinoma is related to poor therapeutic response,and multimodality therapy may improve the prognosis.

ABSTRACT

OBJECTIVE:To study the preventive effect of Lonicera japonica alcohol extract on mice with liver injury based on metabolomics method. METHODS:30 mice were equally randomized into a normal control (isometric normal saline) group,a model(isometric normal saline)group and a group of L. japonica alcohol extract(2 g/kg). The mice were given drugs by ig once a day for 14 consecutive days. On the 8th day of administration,the models were established by giving 0.2% dimethyl sulfoxide (DMN,10 ml/kg)ip once a day for 7 consecutive days. Gas chromatography-mass spectrum(GC-MS)was used to analyze 24 h total ions chromatogram of the urine sample on the 1st,3rd,5th and 7th day of administration of drugs and DMN. The change in endogenic small molecule metabolites in urine was observed. Principal component analysis was employed to explore the change in the metabolite chromatogram and underlying biomarkers in urine. RESULTS:The contour of the chromatogram changed to a largest extent 1 to 5 d after given DMN,but showed an obvious trend towards regression 7 days thereafter. DMN resulted in increase in the contents of 8-phenyl-8-azbicyclo-[4,3,0]non-3-ene-7,9-dione,2-(6-heptynyl)-1,3 dioxolane,bis-(O-methyloxime)-4-ketoglu-cose,and decrease in the contents of malonic acid,2-(4- chlorophenylthiomethoxyl)ethyl,tetrahydro-2-furanacetaldehyde,D-ga-lactose,erythro-pentonic acid and galacturonic acid,in endogenic small molecule metabolites in mouse urine,for which Lonicera japonica alcohol extract can improve that. CONCLUSIONS:Previous administration of L. japonica alcohol extract ig has preven-tive effect to some extent on the physiological and metabolic conditions of mice with liver injury induced by DMN.

ABSTRACT

Objective To compare the effects of thoracoscopic 2 hole and 3 hole for congenital pulmonary bulla resection.Methods 38 cases of congenital pulmonary bulla patients,both in the VATS downlink congenital pulmonary bulla resection.According to the number of holes,thoracoscopic operation were divided into the two groups. To observe the use of group 19 cases of 2 holes,19 cases 3 holes were adopted in the control group.Pull the chest tube operation time,operation time of the two groups were compared after.and the average hospitalization time after opera-tion,postoperative analgesia drug application.Results The observation group operation time,operation time,pulling the chest tube after operation the average hospitalization time, analgesic drug application rate respectively were (46.89 ±9.11)min,(3.95 ±0.85) d,(7.37 ±1.34) d,21.1%,The control group were (66.05 ±12.09) min, (4.37 ±0.98)d,(7.32 ±1.57)d,52.6%.There were statistically significant differences in rate of the two groups in operation time,analgesic drug application.(t=-5.516,χ2 =4.071,P<0.05);38 cases were cured,followed up for 3-24 months,no recurrence occurred in 1 cases.Conclusion Video assisted thoracic descending congenital pulmona-ry bulla resection,the 2 hole 3 hole more than minimally invasive,short operation time,postoperative analgesic use rate is low.

ABSTRACT

The mechanism of biological actions of quercetin was studied by using metabolomic method and biomolecular network. HPLC-MS was used to analyze the serum metabolome in rats of blank group and quercetin administration group rats, and MS data were processed by MATLAB software. With multivariate statistical analysis of serum metabolite profiles, a clear separation among blank group and quercetin administration group was achieved, potential biomarkers were selected according to the parameters of variable importance in the projection (VIP) and identified according to MS information and database retrieval. Four compounds, related enzymes, action targets and metabolic pathways had been confirmed, namely retinoic acid and RARbeta, arachidonate and COX-2, 3, 5-diodotyrosine and TPO, uridine diphosphate glucose and PDEs. The mechanism of quercetin enhancing ability of retinoic acid on the induction of RARbeta, activating TPO, using as COX-2 and PDEs inhibitor was approved by biomolecular network and related literatures. In this study, a mechanism of multiple biological actions of quercetin was evaluated at the level of the biomolecular network, metabolomics and biomolecular network can be used to investigate the biological effects mechanism of quercetin, which provided a new method to further revealing mechanism of drug action.

ABSTRACT

Objective To investigate the clinical feature of perioperative management of non-small cell lung cancer(NSCLC) in aged patients and improve the efficacy of surgical treatment.Methods The clinical data of 35 aged patients with NSCLC were retrospectively analyzed.The risk factors of postoperative complications were analyzed by single factor analysis,the factors had statistical significance were included in Logistic regression analysis.Results Postoperative complications occurred in 10 cases,accounting for 28.6%,and 1 case died,accounting for 2.9%.Logistic regression analysis showed that smoking,chronic bronchitis,coronary heart disease,pulmonary lobectomy were independent risk factors of postoperative complications.ConCLusion Correct staging before operation,strict surgical indication,choose the standard surgical method to reduce surgical trauma,strengthen the perioperative management can still achieve satisfactory therapeutic effect in surgical operation for aged patients with NSCLC.

ABSTRACT

<p><b>OBJECTIVE</b>To study the optimum technical conditions of extracting Hydroxysafflor yellow A (HSYA) from Carthmus tinctorius by multi-stage countercurrent extraction technology.</p><p><b>METHOD</b>The effects of extraction time of each stage, extraction temperature, ethanol concentration and solid-liquid ratio (g x mL(-1)) on extraction yield of HSYA were studied by orthogonal test design and the comparison of other extraction methods were presented.</p><p><b>RESULT</b>Extraction time and solid-liquid ratio had significant influence on the extraction yield, and the optimum parameters were as follows: Extraction time of each stage was 120 min, solid-liquid ratio was 1 : 10 (g x mL(-1)), ethanol concentration was 30%, and extracted at room temperature. Under the optimum conditions, the extraction yield of HSYA was 1.56% and the purity of the extract was 6.06%. Compared with the traditional extraction method and the ultrasonic extraction method of the pharmacopoeia, the extraction yield was increased by 6.12% and 9.09%, the purity of extract was increased by 42.9% and 27.0%, respectively.</p><p><b>CONCLUSION</b>The multi-stage countercurrent extraction technology has many advantages such as simple operation, less solvent consumption, higher extraction yield and purity of extract and it has wide industrial application prospect.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To set up a novel method for fast analysis of active components in water extracting process of Paeonia lactiflora with near infrared spectroscopy (NIRS).</p><p><b>METHOD</b>HPLC was used as the reference method to determine the content of Paeoniflorin and a multivariate calibration model based on PLS algorithm was developed to analyze the correlation between the spectra and the corresponding values determined by the reference method.</p><p><b>RESULT</b>The correlation coefficient of the calibration model was 0.996 2, and the predicted coefficient was 0.989 5. The RMSEC and RMSEP were 0.109 g x L(-1) and 0.138 g x L(-1), respectively, and the RSEP was 5.6%.</p><p><b>CONCLUSION</b>The method mentioned above is proved to be convenient, rapid and nondestructive, accurate and reliable, and is applicable for fast analysis and monitoring of active components in extraction process of traditional Chinese medicine.</p>
